The following forms must be completed before a student may participate in athletics:

• Parent/Student Athletic Agreement – Required for each athlete, each sport season
• Athletic Early Release Transportation Form – Required for each athlete, each sport season
• Catholic Schools Office Medical Physical Form – Required annually for each athlete and must be completed and signed by a physician, indicating the student is cleared to participate in athletics

Athletic Fees

The non-refundable athletic participation fee is $280 per athlete, per sport.

For questions regarding athletic participation, required forms, or fees, please contact the school office.

A current sports physical is required for all students participating in athletics and must be completed by a licensed healthcare provider.

Sports physicals may be obtained from:

• Your child's pediatrician or family physician
• An urgent care clinic or walk-in medical provider
• Any qualified healthcare professional authorized to perform sports physical examinations

The completed physical form must be submitted to the school before a student may participate in practices or competitions.
